McConnell Receives AGS Award

Congratulations to Eleanor McConnell and the entire research team who received an award from The American Geriatrics Society (AGS) for her proposal entitled "GWEP-CC Innovative Geriatrics Programs and Approaches to Care." This project was awarded $7,500 for the one-year project period to begin Dec. 1, 2017.

This award is to support consultation with Community Catalyst, Inc., focused on strengthening consumer voice as we support development of a Durham CTC will greatly enhance our ability to meaningfully engage a diverse cross-section of older adults and their family caregivers with GWEP partners. This team will use consultation from Community Catalyst to map the ecosystem of care transitions in Durham County, assess communications that various organizations in that ecosystem have for connecting with each other and with consumers, and identify gaps in information that could be addressed through more consumer-oriented communications. This consultation will undergird the development of a transitions coalition that works effectively for both providers and consumers and that can be sustained over time. 

We envision that the CTC will help older adult consumers and their caregivers in Durham County raise their voice and contribute to shaping strategies and services to promote safe and effective transitions. Older adults and caregivers would have access to materials that would guide them to advocate for high quality transitions---including questions to be asked before and after discharge, contact information for community agencies that can help with care and resources during transitions, and reports on the performance of health systems and practices on avoiding readmissions or other related problems.
