What is Extend?

EXTEND is a research study at Duke University Health System. The goal of the study is to help adults who have difficulty managing their type 2 diabetes and high blood pressure improve their health through the use of technology and monitoring.

Is EXTEND a Good Fit for You?

EXTEND may be a good fit if you:

• Are between the ages of 30-75
• Have difficulty managing your diabetes (HbA1c ≥ 8.0) and blood pressure
• Have a smartphone with a data plan (Apple or Android)
• Are a patient at one of the following clinics: Triangle Family Practice, Duke Primary Care Mebane, Duke Endocrinology Clinic 1A, Duke Endocrinology South Durham, Duke Outpatient Clinic, and Duke Endocrinology Brier Creek

What is involved?

You will be randomized (like a coin toss) to participate in one of two programs: EXTEND or EXTEND Plus. The study will last for 24 months (2 years). Program descriptions and activities are below.
