Donna Biederman presented on "Implementing a Homeless Transitional Care Program" at the National Health Care for the Homeless Conference and Policy Symposium in Portland, Ore. Biederman was part of a panel on Strengthening Care Coordination & Transitions in Medical Respite Care for the Council's pre-conference institute. Her presentation featured the work made possible by the Hillman Innovations in Care Award.
