The deadline for employees to obtain their flu vaccination is fast approaching (Tuesday, November 5)! Remember, effective this year, flu shots are required of ALL DUSON faculty, staff, clinical instructors, clinical nurse educators, seminar leaders, other part-time instructors, and post-doctoral fellows regardless of whether or not they would be present in a clinical settings, or come into contact with patients or research subjects.

Exemptions: Applications for exemptions were due to Duke Employee Occupational Health and Wellness by Monday, October 14.

Received vaccination outside of Duke: If you were vaccinated outside of Duke Employee Occupational Health and not by the Duke peer vaccination program, submit official documentation of flu vaccination to http://sites.duke.edu/vaxtraxdoc. Official documentation must include your Duke Unique ID, and any of the following:

• Print out from MY Chart showing employee name, date and type of vaccine administered.

• Documentation from provider with clinic letterhead/address or encounter form showing date and type of vaccine administered, date of birth.

• Pharmacy printout with employee name, date of birth, name of vaccine, date of vaccination and location/address of pharmacy.

• Your Duke Unique ID must be written on the flu documentation to ensure that the correct employee gets the correct credit. Also, add your best contact phone number in case EOHW needs to reach you.

New Process to Register for Flu Vaccination: This year, Duke has a new, electronic process for flu vaccination registration. This process will replace the paper forms used in previous years. Bring your smartphone, if you have one, for faster registration. You can bring your smartphone to access the fast pass registration line; use your phone's camera to scan a OR code on-site or visit a special URL to complete the registration; if you do not have a smartphone, you can use a device provided at the vaccination site. Wait times may be longer with this option.

Vaccination Schedules for Staff and Faculty: Faculty and staff can visit Employee Occupational Health & Wellness or attend a roving clinic for flu shots. No appointment is necessary. Visit this link to for the schedule.
