Duke Community & Family Medicine: Grand Rounds

Duke Community & Family Medicine is sponsoring a grand rounds event, From the Medical Home to the Medical Neighborhood: Primary Care Meets Population Health. This event will be on Thurs. Nov 9 from 12-1 p.m. in room 131 at Hanes House. This event is presented by Kevin Grumbach, MD. At the end of the presentation, learners should be able to:

• Define the 10 building blocks of high-performing primary care
• Understand the relationship between medical homes and medical neighborhoods
• Identify at least 4 pragmatic ways to integrate a population health approach into primary care practice
• Recognize why academic health centers embarking on ACO and other risk-sharing financing models will not succeed without a strong foundation of primary care
