DUSON Welcomes PCCP Fall 2020 Virtual Cohort

pccp zoom meetingDUSON would like to welcome the Population Care Coordinator Program (PCCP) Fall 2020 virtual Cohort from Visiting Nurse Service New York (VNSNY). For over 125 years, the Visiting Nurse Service of New York has been committed to meeting the health care needs of New Yorkers with compassionate, high-quality home health care.

Partnering again with Duke University PCCP program, these Duke-trained population care coordinators will be able to identify emerging trends in patient populations and create opportunities for those patients to more successfully manage their chronic and complex health issues, avoiding costly emergency room visits and hospital readmissions.

The Duke University PCCP program is geared towards RNs with a BSN but having a BSN is not required in order to take this course. It is an intensive, 13-week online course (two didactic and one clinical immersion of 160 hours) and requires approximately 10-14 hours per week for course work.

Members of the 2020 fall cohort are: Eunice Reynoso, Monica Cayemitte, Anne McGrath, Karen Ciardullo, Chantal Jean-Peaul, Jacqueline Kassman, Lynette Ramlogan, Keisha Smith, Hui Xiao, George Bosch, Gale Halley, Sandra Shau and Ladz Cheriel-Burke.
