DUSON Welcomes Population Care Coordinator Program Summer 2019 Cohorts
Duke University School of Nursing welcomes its newest Population Care Coordinator Program summer cohorts. The goal of the Population Care Coordinator Program is to improve patient outcomes by helping medical care teams operate more efficiently and effectively. Duke-trained Population Care Coordinators can identify emerging trends in patient populations and create opportunities for those patients to more successfully manage their chronic and complex health issues, avoiding costly emergency room visits and hospital readmissions.
Orientations were held on April 25-26 on location at Visiting Nurse Service New York (VNSNY) and the Army representative’s orientation was held on May 23-24 at the JB Duke Hotel and Convention Center at Duke University.
