Turner Submits Duke CTSI Population Health Improvement Co-Development Application

Kudos to Barbara Turner and Co-PIs Cheryl Brewer and Camille Grant and their entire team for the submission of her Duke CTSI Population Health Improvement Co-Development application entitled "Assessing Baseline Health Literacy: In Medically Underserved Patient with Hypertension" with the Lincoln Community Health Center as a community partner. This proposal requests funding for a one-year period with a start date of June 1, 2018.

Health literacy impacts care related to medical decision making, access to care, healthcare utilization and health outcomes. Understanding medical information is more daunting for medically underserved populations with illnesses such as hypertension. In order to provide optimal care for patients by making them equal partners in their care, the Partnership for a Healthy Durham’s Access to Care Health Literacy subcommittee and Lincoln Community Health Center (LCHC) will assess health literacy in a subset of 100 medically underserved patients with hypertension in Durham County. The study will last from three to six months with analysis and follow up for six months.

To achieve our intended goal, this team will secure approval from the appropriate institutional review board and engage in the following activities during the proposed pilot:

Activity #1 - Design assessment

o Identify hypertensive patient population at LCHC (>=100 patients)

o Select health literacy tools to test

o Identify pre- and post-assessments

Activity #2 - Enroll participants

o Design marketing materials

o Spend $300 by month one on marketing

o Purchase incentives

o 50% of participants educated on pilot and enrolled by month two goal

o 100% of participants educated on pilot and enrolled by month three

Activity #3 - Implement pilot assessment

o Administer pre-assessment to 100% of participants by month four

o Administer post-assessment to 100% of participants by month eight

Activity #4 - Data Analysis

o Analyze pilot data and identify discrepancies by month 10

o Develop focus group protocol for follow-up

Activity #5 - Participant Follow-Up

o Recruit for focus group

o Schedule and facilitate focus group

o Data analysis completed by month 12
