At the end of the lecture, participants will be able to:
  • Discuss the role of the nurse in leading population health improvement initiatives.

  • Describe the knowledge and skills necessary to impact the health of populations in practice, research, and policy in the era of value-based care.

  • Identify current and potential evaluation metrics that demonstrate practice outcomes and population health improvement outcomes.

  • Explain the use and application of Social Determinants of Health assessment skills across practice settings.

Who Should Attend

The lecture will focus on education for health care providers (including nurses, social workers, therapists, physician assistants, physicians and others) and educators, community members and medical and nursing students.

Keynote Speaker

Dr. Susan SwiderSusan Swider, PhD, PHNA-BC, FAAN
Director, Advanced Public Health Nursing Program & Transformative Leadership: Population Health Program
Professor, College of Nursing, Rush University

Dr. Swider is a professor in the Department of Community, Systems and Mental Health Nursing at Rush University. She has practiced in acute care, home health and public health settings over the past thirty years. Her research has focused on program development and evaluation of community health workers, engaging urban communities in health promotion, and health policy to support health promotion efforts. She has taught public health nursing and co-directed 10 years of HRSA supported work to develop an online doctoral program in Advanced Public Health Nursing, ensuring that it met the Quad Council competencies for Public Health Nursing practice. She belongs to a number of public health/nursing organization, and is past president of the Association of Community Health Nursing Educators (ACHNE).

In 2011, Swider was appointed by President Obama to the Advisory Group on Prevention, Health Promotion and Integrative and Public Health, to advise on the development and implementaiton of the National Prevention Strategy. She is a fellow of the American Academy of Nursing and the Institute of Medicine of Chicago. In 2016, she was appointed to serve on the community Preventive Task Force of the Centers for Disease Control and Prevention.
